The ID submitted must be the same as the ID number entered above in the "Requestor's Driver's Licence or other State Issued ID" fields. See below for the list of acceptable IDs.

IDs marked “not for federal use” are NOT accepted by the state.

In the event that your certificate cannot be located, state law requires a search fee equal to the certificate cost be collected. Search and service fees are non-refundable or transferable. The shipping fees may be refunded.
